Request Repeat Prescriptions?
The medical centre is computerised. Prescriptions are printed together with a
re-ordering slip. When you require more medication, tick the items required on the
slip and return it to the medical centre, allowing at least two working days before
collection. If you are taking regular medicines you must be seen by your doctor at
least once a year. In conjunction with local chemists, we have set up a collection
service. This means that instead of coming to the medical centre to collect your repeat prescription, then going to the chemist and waiting for your medication to be dispensed, you can go straight to your chemist who will have your medication ready and waiting for you. In this case, please allow three working days for the chemist to collect your prescription and prepare your medication. On your repeat prescription slip there is a space to tick if you would like a chemist to collect your prescription. Some chemists
also offer a home delivery service.
A faxed prescription is not an acceptable legal document. Prescriptions will only be faxed to chemists in exceptional circumstances.
How Do I....
Obtain A Sick Note?
For a short illness (less than seven days) you should complete a self certification form which is available from your employer. All other medical certificates need verification
and you will need to make an appointment to see the doctor. There are charges for non-NHS certificates such as sick notes, passports, insurance claims and diving medicals.
