Register With The Practice?

Please bring your medical card when first registering with a doctor. You will also be asked to complete a simple questionnaire. You will be registered with the practice but we will respect your wish to see a doctor of your choice. Whilst the practice will endeavour to comply with this it might not always be possible within 48 hours. Patients over five years old will be invited to see the nurse for a registration medical. The nurse will ask you some questions about your health, then check your height, weight and blood pressure. This gives us basic information about you until we receive your medical cards from your previous doctor. Please bring a fresh urine sample in a clean container and details of any regular medicines that you are taking.

How Do I....
Change My Details?

If you change your name or move, PLEASE let the surgery know as soon as possible.

If you have a telephone number or if you change it, please let the medical centre know, especially if it is an ex-directory number.
